What to Bring to the Consultation
After you make an appointment and at least 24 hours prior to the consultation, you should fax or email us the The Anderson Firm Potential Client Profile for the relevant practice area and any of the following documents in your possession, depending on the type of case. If you do not have some of the documents in your possession, just let us know during the consultation.
Divorce
All documents that have been filed with the court by you or by your spouse for this case and any related case, including:
- any bankruptcy filed during the marriage
- all letters, emails or faxes from your spouse’s attorney, if any
- tax returns, W2s, 1099s and other documents evidencing your and your spouse’s sources of income for the past three (3) years (unless the marriage was shorter, then the time period of the marriage)
- the most recent 401K, IRA, retirement or pension plan statement for you and your spouse
- marital home records (mortgage statement(s), line of credit statement(s), lease agreement, tax assessment, appraisal, etc.)
- the most recent bank and investment statement for you and your spouse jointly and individually
- court orders for which you and/or your spouse are still responsible
- your completed Domestic Relations Financial Affidavit
- evidence such as pictures, recordings, police reports, hospital records or other evidence that support what you are alleging such as adultery, mental cruelty, domestic violence, alienation of affection, financial responsibility and irresponsibility in finances, with the children or in the marriage
Child Support, Modification, Legitimation and Custody Cases
- Your completed Domestic Relations Financial Affidavit
- all documents that have been filed with the court by you or by your spouse/other parent for this case and any related case, including abandonment, protective orders, etc.
- evidence such as personal logs, pictures, recordings, police reports, hospital records, school records or other documentation that support what you are alleging such as income being paid “under the table,” unpaid health/dental insurance co-pays/bills, abuse, plans to relocate/relocation, domestic violence, alienation of affection, financial responsibility and irresponsibility financially, or any other actions or communications that you deem not to be in the best interests of the children
Contempt
Evidence you have showing that the current order is not being followed.
Prenuptial, antenuptial and cohabitation agreements
- Your completed Domestic Relations Financial Affidavit
- tax returns, W2s, 1099s and other documents evidencing your and your spouse’s sources of income for the past three years
- your most recent mortgage, car note, investment(s), 401K, IRA, life insurance, disability insurance, retirement or pension plan statement
- a list of all of your pre-marital/pre-cohabitation assets and liabilities
- a copy of the title to all real and personal property you own individually or jointly
- documentation evidencing your interest/ownership in any business
- any proposed or current agreement for which you seek representation
- the resume and business card used by you and your significant other
- your will
- your insurance policies
- court orders for which you or your significant other is responsible
- any documentation or information evidencing any trust, inheritance, annuity, prize monies or proceeds from a judgment you are entitled to or are expecting
- a description of no more than one page of your goals and objectives for this matter
Wills
- A description of no more than one page of your goals and objectives
- a list of all of your assets and liabilities, including personal and real property that you seek to bequeath
- your most recent mortgage, car note, life insurance, investment(s), 401K, IRA, retirement or pension plan statement
- a copy of any existing will or codicil that you have executed
Entertainment
- The proposed or current contract, memorandum of understanding or letter agreement for which you seek representation
- the full, legal and professional names of all contractual parties on the other side, their titles, company affiliations, management and contact information
- a description of no more than one page of the entertainment matter or issue for which you seek representation
- a copy of the mark you seek to own or trademark, if applicable
- evidence such as logs, and publishing documentation
- invoices, judgments or royalty statements that have not been paid
- evidence you have showing that the contract (including any amendment or addendum) has been breached, if applicable
Business
- The proposed or current contract, memorandum of understanding, deal memo or letter agreement for which you seek representation
- a description of no more than one page of the business or transactional matter or issue for which you seek representation
- a copy of the mark you seek to own or trademark, if applicable
- for start-ups, joint ventures, corporations and limited liability companies evidence you have showing that the contract (including any amendment or addendum) has been breached, if applicable
- invoices or judgments that have not been paid
- if the company is already in existence, the articles of incorporation or organization, the bylaws or the operating agreement, including all amendments, and the names, titles and contact information for all partners, owners, members, shareholders and profit participants
